During an interview on 8/26/2025 at 1:48 AM with the surveyor the Director of Nursing, in the presence of the Licensed Nursing Home Administrator Regional Clinical Director acknowledged that the MAR/TAR should be completed in its entirety upon completion of the order and that if it was not filled in it could mean that the order was not done. A review the undated facility policy titled, Administering Medications revealed the following under Policy Interpretation and Implementation: [ .]
- Medications must be administered in accordance with the orders, including any required time frame.

The individual administering the medication must document in [electronic records] after giving each medication by clicking the Y. [ .] 13. If a drug is withheld, refused, or given at a time other than the scheduled time, the individual administering the medication shall click the N on the EMAR and then document as prompted by PCC. A review the undated facility policy titled, Charting and Documentation revealed the following under Policy Interpretation and Implementation: 1.
