The other residents who witnessed it were scared enough that they waited until the next day to report what they had seen.

The resident had undergone cardiac surgery.

That was the finding when a complaint inspection landed at the facility on August 9, 2025.

What inspectors found was a kitchen operating out of compliance with rules the facility had written for itself.

The inspection, triggered by a complaint, resulted in a citation for actual harm.

Nobody told the medical director it had happened.

The resident, identified in inspection records only as R55, lives at Green Acres Health and Rehabilitation on Allen Memorial Drive.

The office was shared with the admissions coordinator and the administrator.

Bread wrappers, discarded packaging, whatever had accumulated, left there while people tried to eat.

Nobody investigated for four days.
