Federal inspectors observed Certified Nurse Aid #128 at 1:10 p.m.

Resident 3 was supposed to receive seizure prevention medication twice daily at 8:00 AM and 8:00 PM.

As of the most recent federal records, the facility has **not submitted a plan of correction** to address the cited deficiencies.

The violations centered on Enhanced Barrier Precautions, the layer of protection required when standard contact precautions don't apply.

A few residents were affected, according to the report.

The resident's doctor told inspectors on October 24 she expected Thermopolis Rehabilitation and Wellness to notify her if blood glucose exceeded 400.

But the assistant denied everything.

The resident, identified in inspection records only as R1, had asked for a bedside commode.

Federal inspectors visited the facility on August 29, 2025, following a complaint.
