Resident 41 had been admitted to Mirage Post Acute on March 3, 2026.

The admission came during a complaint inspection on April 24, 2026.

The woman's care plan reflected none of it.

The resident at the center of the lapse, identified in inspection records as Resident 3, had been living at the facility since August 2024.

The deficiency was one of nine cited during the April 24 inspection, which was triggered by a complaint.

The resident, identified in inspection records only as Resident 1, had been prescribed Hydrocodone on a as-needed basis, every four hours.

The incident happened on April 17, 2026.

Inspectors classified it as an isolated violation.

That sequence, documented by state inspectors at Cypress Ridge Care Center following an April 2026 complaint investigation, played out more than once.

The resident had trauma in her past.

She then walked with the inspector to the bathroom and was asked to demonstrate.

The resident told inspectors that staff dressed the wound for only the first couple of days.
