That is what federal inspectors found when they arrived at Rose Lane Nursing and Rehabilitation on January 29, 2026, responding to a complaint.

What replaced it was a lavender-scented all-purpose cleaner called Medorra Limpreza.

She was dependent on staff for everything — every activity of daily living, according to her care plan.

The citation, issued under the category of resident rights deficiencies, was one of 13 separate deficiencies inspectors documented during the visit.

That was one moment during a single lunch service on May 18.

The resident, identified in inspection records only as R1, lives at Ascension Nazarethville Place, a nursing facility at 300 North River Road in Des Plaines.

What replaced it at Grove at Kirkwood was a lavender-scented all-purpose cleaner called Medorra Limpreza.

The director of nursing, when interviewed by inspectors, did not dispute what had happened.

The citation, issued April 24, found that the facility failed to provide safe and appropriate pain management to a resident who required those services.

State health officials received no written report.
