Federal inspectors found the facility failed to complete the required care plan for Resident #2 within 21 days of her October 6 admission.

On October 22, inspectors observed Resident 43 sitting in dining room B at 12:15 PM, waiting for lunch trays that didn't arrive until 1:10 PM.

At least six residents developed severe itching rashes consistent with scabies infestation by mid-October.

The resident had called her son on September 1st asking him to bring a fly swatter because there were so many flies in her room.
