Inspectors from the Centers for Medicare and Medicaid Services cited Presidential Post-Acute following a complaint investigation completed October 20, 2025.

Inspectors visited Avir at Kingsland on November 11, 2025, following a complaint.

The allegation involved Resident #2.

The receipts, reviewed by inspectors as of July 14, 2025, were effectively blank on every field that would establish who took custody of the drugs and when.

The machine suspends a person in a fabric sling, lifting them off one surface and swinging them to another.

That gap is what inspectors found when they visited the facility on October 21, 2025.

The facility's own written policy said what was required.

Inspectors determined that a few residents were affected and that the level of harm was minimal or represented potential for actual harm.
