Federal inspectors documented both medication errors during an August complaint investigation at the facility on Frederick Road.

Staff C worked multiple hallways during her shift but managers considered the incident "isolated" without investigating further.

The resident's care plan, initiated June 24, specifically required "moderate assist of 2" staff members along with a rolling walker and gait belt for safety.

The incident occurred at MacLay Healthcare Center on August 23 at 6:30 a.m.

The patient, identified as R12 in inspection records, had tested positive for Clostridium difficile on August 18.

The incident came to light when two nursing assistants attempted to move the resident up in their chair.

No soap or other approved cleaning product was used during the procedure.

Northgate Unit of Lakeview Christian Home filed an annual assessment claiming the resident had not fallen since admission.

But a 30-day review revealed a pattern of missed care.

Three nursing assistants witnessed the incident and confirmed the nurse skipped the floor assessment.

The failure violated federal safety requirements designed to track controlled substances and protect residents from medication errors.
