The January incident was one of several reporting failures discovered during an August inspection at the 3057 Gull Road facility.

They required one-person assistance for toileting and transfers, plus supervision for eating, walking and bed mobility.

The physical therapist's documentation of the incident should have triggered immediate reporting to administrators under federal nursing home regulations.

The incident occurred during routine personal care at Excel Care at Manalapan when CNA #1 and CNA #2 were changing Resident #43.
