For several of those missed checks, staff had written that the resident was asleep.

Her care plan, written in April 2024, directed staff to perform passive range of motion exercises on her at bedtime, ten repetitions each night, every night.

Immediate jeopardy is the most serious classification available to federal inspectors.

It means the problem was not isolated to one room or one resident.
