The incident happened on September 9, 2025, at Mulder Health Care Facility, a nursing home on Leonard Street in West Salem.

It is not a finding reserved for close calls.

And they had come from inside a nursing home that, at the time of the inspection, was caring for 77 residents.

One of them, identified in inspection records only as Resident 10, was moving in a way that alarmed a federal inspector watching the scene unfold.

Both described her the same way.

The licensed practical nurse was hired January 8, 2025.

The facility's own written policy described exactly what should happen in those moments.

Let it go, the assistant director of nursing and the assistant administrator told her.

Together they reflect a gap between what staff said the rules were and what inspectors found when they walked the halls.

The administrator said she couldn't verify whether the check had ever been run.

Dust had built up on all baseboards on all units.

That was one of the smaller problems inspectors found at Optalis Health & Rehabilitation of Bloomfield Hills during a September 17, 2025 complaint inspection.
