That was on the night of August 10th.

The records, covering shifts from July 21 through August 10, showed one nurse clocked in at 6 p.m.

The resident had undergone cardiac surgery.

That was the finding when a complaint inspection landed at the facility on August 9, 2025.

What inspectors found was a kitchen operating out of compliance with rules the facility had written for itself.

The inspection, triggered by a complaint, resulted in a citation for actual harm.

Nobody told the medical director it had happened.
