That was one of three separate abuse incidents investigators documented at the 6525 Lancaster Pike facility.

Inspectors were there at 10:17 in the morning on May 23, 2026, and they watched it happen.

The inspection, conducted April 24, 2026, found that Resident 7 was receiving wound care but that the care plan contained no information about it.

That's what state inspectors found during a May 26 complaint inspection at the 31741 Rancho Viejo Road facility.

Six of 33 sampled residents described the same problem.

A physician had ordered the medication in July 2025.

Inspectors visited the Portland nursing facility on April 24, 2026, responding to a complaint, and left with nine deficiencies on the books.

Three ventilators were involved in the resident's care between March 2 and March 14.
