interview on 10/01/2025 at 4:36 PM with the P.ADM confirmed that with the diagnoses Resident 6 had of CHF, CKD, and a surgical repair of a right ankle fracture, assessments should have included shortness of breath and O2 sats, edema, and NV status of the right foot and that the EHR did not include daily assessments of those.An interview on 10/01/2025 at 4:36 PM with the Director of Nursing (DON) confirmed that with the diagnoses Resident 6 had of CHF, CKD, and a surgical repair of a right ankle fracture, staff should have assessed the resident for respiratory status including shortness of breath and O2 sats, edema, and NV status of the right foot at least daily. The DON confirmed the EHR did not include daily documentation of those assessments and that the required daily Medicare charting was not present.
