I have a supra pubic catheter, but I wear a diaper for my bowel movements. They run out all the time, and they run around different rooms trying to find me one when I need to have a bowel movement. I prefer having a brief on at all times. I don't like it when

they try to do the sheet between my legs.Record review of the admission Record revealed Resident #3 was admitted to the facility on [DATE REDACTED] with medical diagnoses that included Polyneuropathy, Atherosclerotic Heart Disease, and Neuromuscular dysfunction of the bladder. Record review of the MDS with an ARD of 10/24/25, revealed under section C, a BIMS summary score of 13, indicating Resident #3 is cognitively intact. Section H- Bladder and Bowel revealed that Resident #3 has an indwelling catheter and is always incontinent of bowel. Resident #4During an interview on 12/30/25 at 9:40 AM, Resident #4 stated, I get nervous and stressed at night when I go to bed. I'm afraid to move and I try to lay as still as I can, so I don't urinate because I know they are always out of diapers. I say something to them, and they say, 'Well, they didn't give us anymore'. Resident #4 revealed that for what I'm paying to be in this facility, they should be providing my diapers. Resident #4 stated, Will you please do something about this? The resident further revealed that she hasn't gone to upper management because I try to get along, and I figured it wouldn't do any good anyway. Resident #4 confirmed that this causes her a lot of mental anguish, not having the incontinent supplies that she needs and the thought of having to urinate on a bedsheet. Record review of

the admission Record revealed Resident #4 was admitted to the facility on [DATE REDACTED] with medical diagnoses that included Spinal Stenosis, End Stage Heart Disease and Anxiety Disorder. Record review of the MDS with an ARD of 10/10/25, revealed under section C, a Brief Interview for Mental Status (BIMS) summary score of 14, indicating Resident #4 is cognitively intact. Section H- Bladder and Bowel revealed that Resident #4 is always incontinent of bowel and bladder.
