During this recertification survey, the facility failed to ensure a resident was free from physical restraints by not assessing for a least restrictive restraint for one (1) of one (1) sampled residents for restraints. During the recertification survey on 7/24/23, the facility failed to obtain a physician order for the use of a restraint for one (1) of three (3) residents reviewed for restraints.

On 8/1/24 at 3:34 PM, during an interview with the Administrator, she revealed that staff had been in-serviced and they performed weekly audits as part of their correction for addressing hazards related to restraints. She also noted that they had a high-risk meeting coming up where the topic of restraints was to be discussed and any problems would have been identified during that meeting.
