interview on 5/28/26 at 3:05 PM, with the facility Administrator revealed that it is his expectation
facility admitted the resident on 4/4/2023 with diagnoses that included Unspecified Dementia,
History of Falling. A record review of the Minimum Data Set (MDS) with an Assessment Reference Date (ARD) of 4/7/2026 revealed Resident #1 was unable to complete a Brief Interview for Mental Status (BIMS).
