the abuse the resident had caused the nursing staff. She was not the decision maker about who the facility would accept, and those decisions are made by corporate staff and the administrator. During an interview
on 8/7/25 at 1:10 P.M the ADON said she was with the resident during the transport to Hospital B to be evaluated for psychiatric needs as the resident had stated he/she was going to hurt his/her-self and others.
The ADON did not feel that the resident's needs could be met by the facility staff and that the resident was
a danger to others. During an interview on 8/7/25 at 4:05 P.M. the Administrator B said the public administrator responsible for Resident #1 was notified on 7/31/25 by the Administrator A that the facility would not be accepting the resident back for the safety of the staff and other residents. She believed it was
in the best interest of the resident, the staff, and the other residents for the resident to be immediately discharged from the facility's secured behavioral unit.During an interview on 8/8/25 at 10:55 A.M Deputy Chief Public Administrator said the resident is still at Hospital B waiting for Hospital B to find appropriate placement for the resident. She does not feel that long-term care setting is the appropriate place any longer. He/she has not been able to find placement for the resident. During an interview on 8/8/25 at 10:05 A.M. Hospital B's RN B said, as of 8/8/25 the resident has no need to be admitted to the hospital and remains at Hospital B in the emergency room pending permanent placement for the resident.During an
interview on 8/11/25 at 1:35 P.M, the Hospital B's RN C said the resident became violent last night towards others and required restraints be applied to prevent the resident from hurting others. The resident was admitted to the hospital and currently is on 1 on 1 monitoring awaiting psychiatric evaluation.During an
interview on 8/11/25 at 1:20 P.M. the Administrator A said on 7/31/25 she notified the public administrator by phone that an emergency discharge was issued for the resident, and the resident would not be allowed back to the facility for the health and safety of the residents and staff.During an interview on 8/11/25 at 1:50 P.M the DON from the sister facility said the resident could become aggressive with staff and would often get involved with other resident's drama or behaviors causing the resident to have behaviors. The sister facility thought the resident could use a change and though the facility would be a good fit for the resident.During an interview on 8/11/25 at 2:35 P.M. the resident said he/she believed that he/she would be going back to the facility after treatment at the hospital. His/her personal belongings are at the facility, and he/she has nothing. He/she had made friends at the facility and missed them and the other residents.
He/she is uncertain about where she will live and what the plans for the future will be and said he/she was frustrated he/she was not able to return to his/her home.Complaint #2579000 and #2580318
