interview on 12/19/25 at 8:55 A.M., CMT F said they have not worn a pager when working the floor for at least three months. It was about the beginning of last summer when staff stop wearing pagers or Administration stopped monitoring for compliance. 3. During an interview on 12/19/25 at 8:00 A.M., LPN E said the staff do not utilize the pagers anymore and it's been a while since they have carried one on their person. LPN E could not say exactly when the staff stop utilizing the pagers as required by the exception.
The wearing of the pagers was enforced regularly in the past and pagers were used, but it had fallen to the way side, and the majority of staff do not wear the pagers as required. LPN E estimated it had been a couple of months ago when having the pagers became less of a focus. LPN E reported no knowledge regarding the stipulation of staff required to have pagers on their person due to the call light system's exception. LPN E also said Resident #1's call light has not functioned properly for a few months, and he/she has fixed it him/herself, but that was temporarily, as it reverts to functioning improperly after some use. LPN E thought there was an electrical short in Resident #1's call light. 4. During an interview on 12/19/25 at 12:28 P.M., the Maintenance Director said the call light system is monitored by an outside agency and there are no internal audits to ensure the system is functioning properly throughout the facility.
In addition, there are no alerts that come to the facility if the system is not functioning properly. The CNAs should be the ones monitoring the lights for proper functioning. Resident #1's call light has been malfunctioning for some time, and he has fixed it a few times. The call light will work properly after applying
a fix but has reverted back to not functioning properly every time. There are pagers available for staff, but
the majority of staff have stopped using them. 5. During an interview on 12/19/25 at 2:40 P.M., the Administrator said she acknowledged there are stipulations per the exception granted to the facility regarding their call light system. Staff should be wearing pagers when they are working the floor per the exception stipulation, and she was not aware staff were not wearing the pagers. They do not have an internal audit system to ensure the call light system is functioning properly, and the DON is responsible for monitoring nursing staff to ensure pagers are being used in accordance with the stipulation in their exception letter granted to them for using the current wireless call light system. 2694682
