During an interview on 9/10/25 at 8:55 A.M., the Administrator said when a resident is admitted /readmitted to the facility, the admitting nurse was responsible for entering the treatment orders into the computer.

Treatments should be documented when the treatments are completed. If a treatment was not competed, staff should document why it was not completed and notify the physician. A blank on the TAR meant it was not signed out.

When residents leave the facility and return to the facility, it should be documented in the progress notes.
