around 5:00 A.M., by the ADON;-The resident rolled out of bed during cares;-The resident had a laceration which required staples and a neck fracture;-NA D turned from the resident to grab peri-care supplies. While
the NA was turned, the resident rolled out of the bed;-CNA G was present in the room at the time if the incident assisting another resident;-She did not know what the care plan indicated for number of staff required for personal cares for the resident;-The care plan should be followed and properly implemented;-NA D should have positioned the resident on his/her back prior to grabbing supplies to prevent the resident from falling;-The MDS Coordinator, DON, ADON, and charge nurses were responsible for ensuring nursing staff are aware of the care plan needs of each resident and implementing the care plan appropriately.Complaint 2669590
