Inspection Findings
discharging to her assisted living facility with husband and family support. DME (durable medical equipment) ordered- N/A During follow-up interview on 11/20/25 at 1:39 pm., the DON stated her only communication with ALD-A was the email response after business hours on 8/20/25.
She did not ever speak to ALD-A.
The DON stated discharge orders for R37 were faxed to the assisted living however the facility is unable to provide proof of the fax transmission and confirmed R37's medical record does not contain signed provider orders.
The DON confirmed a DME order for the hospital bed was not written. A policy titled Discharging a Resident revised 11/11/20 indicated The resident's attending physician is required to write a discharge order to include medication disposition. It continues, The discharge summary contains necessary medical information that the facility must furnish at the time the resident leaves the facility, to the receiving provider assuming responsibility for the resident's care after discharge.
Further, the policy indicated the summary will include but is not limited to, the following:A. a recapitulation of the resident's stay that includes. bu8t is not limited to, diagnoses, course of illness/treatment or therapy, and pertinent lab, radiology, and consultation results.B. A final summary of the resident's status to include items: .medications, special treatments and procedures.C.
