fairly new admit to the facility and had minimum interventions in the care plan. The DON stated that it was their understanding that following dinner Resident R701 was placed at a table near the nurse's station on the 200 hall. They did not have an exact time of when the resident was placed there and noted there were no cameras on the floor that would confirm timing. The DON then noted at some point in the evening Resident R701 moved themselves from the table to the 100 hall and was found by Nurse B trying to get out of their chair and moved to a different area by the TV. Following the move by Nurse B, the resident had an unobserved fall. The DON was asked about the transfer of Resident R701 following the fall as notes by Nurse A describe the resident as grimacing and grabbing their right lower extremity. The DON stated that staff believed yelling was their baseline did not believe resident was in pain. As such, staff took vitals and utilized a mechanical lift to get them up in their wheelchair and then into bed. The DON was asked was asked why at 9:38 PM it was noted Resident R701 was given both Xanax and Oxycodone . The DON was not able to provide an answer as to whether the PRN medications were given prior to or after the fall and the reason for administering the medication. Subsequent information provided by the Administrator noted the medication was given following the fall indicating PRN Xanax is used for sundowning symptoms and yelling after the resident was laid in their bed. As for the PRN Oxycodone, no specific details as to why pain medication was given were noted.
