The investigation reflected the allegation of choking could not be
readmitted [DATE], with diagnoses that included chronic diastolic (congestive) heart failure, diabetes and unspecified dementia.
The Quarterly MDS, with an ARD of 3/3/26, reflected R4 scored seven out of 15 (severe cognitive impairment) on the BIMS.
According to a facility investigation, on 2/5/26, former Nursing Home Administrator (NHA) K self-reported that staff alleged he yelled/raised his voice at a resident.
According to the facility's investigation, the interview with former NHA K reflected it had been reported to former NHA K that R4 had taken another resident's wallet, with money in it.
The nurse and CNAs tried to get the wallet from R4, but he would not allow it. It was reported R4 was in his room, hollering, swearing and making a big commotion, and staff needed former NHA K to go see R4.
Social Services Assistant (SSA) C, former NHA K and Scheduler G entered the room of R4, who was upset, cursing and hollering at the top of his voice, according to the investigation. R4 kept saying it was his money.
After trying to get R4 to speak with staff, and his constant hollering, former NHA K raised his voice at R4 and said, [R4] quit hollering, we don't need to be hollering.
According to the statement, that calmed the situation enough so staff could continue.
Scheduler G asked R4 about the wallet, and R4 said it was his wallet, his money, and he had it for a long time.
According to the investigation statement, two CNAs were asked to enter the room due to being familiar with R4.
The statement reflected former NHA K opened the wallet, removed $24, which he counted in front of SSA C. R4 stated it was his f**king money.
Former NHA K gave R4 the money, which he put in his pocket.
According to the investigation, it was in fact R4's wallet that was in his possession. In an interview on 3/25/26 at 11:10 AM, Scheduler G reported that during the interaction pertaining to the wallet, R4 was swearing at former NHA K and calling him a mother f**ker.
Scheduler G reported former NHA K turned around and called R4 a mother f**ker.
Scheduler G reported she grabbed former NHA K's arm and tried to guide him out of the room, twice. In an interview on 3/25/26 at 11:33 AM, CNA H reported observing the interaction between former NHA K and R4 from the doorway of R4's room, pertaining to the wallet. CNA H stated R4 called former NHA K a name.
Former NHA K got in R4's face, pointed, told R4 he was not going to talk to him like that, then called R4 a mother f**ker. CNA H stated former NHA K seemed mad, and his face turned red. In a phone interview on 3/25/26 at 1:05 PM, former NHA K denied using profanity or calling R4 names. NHA K denied being frustrated during the interaction with R4 and just raised his voice to stop R4 from hollering. In a phone interview on 3/25/26 at 2:42 PM, CNA I reported directly witnessing the interaction between former NHA K and R4 pertaining to a wallet. CNA I stated she could not recall all of what was said, but former NHA K definitely called R4 a mother f**ker. On 3/25/26 at 1:40 PM, R4 was observed sitting in a wheelchair, in his room.
When queried about his recollection of an incident involving a wallet, R4 stated I had it, and the man thought I stole it. R4 reported swearing at the man, who then came back and swore at him.
When describing the interaction, R4 began raising his voice. R4 reported he forgot what the man said to him but reported cussing words were used towards him.
When asked if he was called any names, R4 stated, cussing words .
