Residents with cognitive loss or impaired judgement may attempt to leave the premises, known as exit seeking behavior.

Residents are most vulnerable in the early admission period; however, residents with progressive cognitive disorders can begin displaying exit seeking behaviors at any time .

Monitoring doors: All exit doors on each unit will be checked to ensure they are locked and secure every shift.

Door security of exit doors will be documented on every shift per nursing assignment sheet.

The designated staff will initial off on the assignment sheet.

Maintenance/or designee will check all doors in the building twice daily.

Any deficient door(s) will be reported to the Maintenance Director immediately. If the alarm system is not in working order, staff will be required to check all doors hourly until the alarm system is up and running .
