Not hire anyone with a finding of abuse, neglect, exploitation, or theft.

Timely report suspected abuse, neglect, or theft and report the results of the investigation to proper authorities.

Respond appropriately to all alleged violations.

Ensure each resident receives an accurate assessment.

Develop the complete care plan within 7 days of the comprehensive assessment; and prepared, reviewed, and revised by a team of health professionals.

Ensure residents do not lose the ability to perform activities of daily living unless there is a medical reason.

Provide care and assistance to perform activities of daily living for any resident who is unable.

Provide appropriate treatment and care according to orders, resident’s preferences and goals.

Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents.

Provide enough food/fluids to maintain a resident's health.

Ensure drugs and biologicals used in the facility are labeled in accordance with currently accepted professional principles; and all drugs and biologicals must be stored in locked compartments, separately locked, compartments for controlled drugs.

Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.

Provide and implement an infection prevention and control program.

Give their staff education on dementia care, and what abuse, neglect, and exploitation are; and how to report abuse, neglect, and exploitation.

Ensure nurse aides have the skills they need to care for residents, and give nurse aides education in dementia care and abuse prevention.

Respond appropriately to all alleged violations.

Honor the resident's right to a dignified existence, self-determination, communication, and to exercise his or her rights.

Reasonably accommodate the needs and preferences of each resident.

Ensure residents have reasonable access to and privacy in their use of communication methods.

Allow residents to easily view the nursing home's survey results and communicate with advocate agencies.

Give residents notice of Medicaid/Medicare coverage and potential liability for services not covered.

Ensure the resident's doctor reviews the resident's care, writes, signs and dates progress notes and orders, at each required visit.

Ensure each resident must receive and the facility must provide necessary behavioral health care and services.

Ensure a licensed pharmacist perform a monthly drug regimen review, including the medical chart, following irregularity reporting guidelines in developed policies and procedures.

Ensure each resident’s drug regimen must be free from unnecessary drugs.

Ensure medication error rates are not 5 percent or greater.

Ensure that residents are free from significant medication errors.

Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.

Provide appropriate treatment and care according to orders, resident’s preferences and goals.

Have a registered nurse on duty 8 hours a day; and select a registered nurse to be the director of nurses on a full time basis.

Ensure a licensed pharmacist perform a monthly drug regimen review, including the medical chart, following irregularity reporting guidelines in developed policies and procedures.

Safeguard resident-identifiable information and/or maintain medical records on each resident that are in accordance with accepted professional standards.

Provide and implement an infection prevention and control program.

Immediately tell the resident, the resident's doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident.

Try to resolve each resident's complaints quickly.

Provide necessary care and services to maintain or improve the highest well being of each resident .

Make sure that the nursing home area is safe, easy to use, clean and comfortable for residents, staff and the public.

Properly hold, secure, and manage each resident's personal money which is deposited with the nursing home.

Reasonably accommodate the needs and preferences of each resident.

Give notice to the resident before a room or roommate change.

Provide housekeeping and maintenance services.

Develop a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.

Allow residents the right to participate in the planning or revision of care and treatment.

Provide necessary care and services to maintain or improve the highest well being of each resident .

Assist those residents who need help with eating/drinking, grooming and personal and oral hygiene.

Give residents proper treatment to prevent new bed (pressure) sores or heal existing bed sores.

Ensure that residents with limited range of motion receive appropriate treatment and services to increase range of motion or prevent further decrease in range of motion.

Give the right treatment and services to residents who display physical or psychosocial problems adapting to changes in circumstances.

Keep the rate of medication errors (wrong drug, wrong dose, wrong time) to less than 5%.

Ensure that residents are safe from serious medication errors.

Store, cook, and serve food in a safe and clean way.

Maintain drug records and properly mark/label drugs and other similar products according to accepted professional standards.

Have a program that investigates, controls and keeps infection from spreading.

Make sure there is a pest control program to prevent/deal with mice, insects, or other pests.

Give residents a notice of rights, rules, services and charges.

Conduct initial and periodic assessments of each resident's functional capacity.

Review or revise the resident's care plan after any major change in physical or mental health.

Ensure each resident receives an accurate assessment by a qualified health professional.

Give or get specialized rehabilitative services per the patient's assessment or plan of care.

Provide routine and emergency drugs through a licensed pharmacist and only under the general supervision of a licensed nurse.
