comminuted left femoral intertrochanteric fracture (hip fracture) and he/she was admitted .During an

interview on 10/07/25 at 3:08 P.M., the Director of Nursing (DON) said she was notified on 08/30/25 by Nurse #1 that Resident #1 was being sent to the hospital for evaluation due to a change in his/her condition. The DON said she interviewed staff and determined that Resident #1 had been lowered to the floor because he/she became shaky and CNA #1 and CNA #2 transferred him/her up off the floor onto the toilet (prior to notifying Nurse #1).The DON said anytime a resident is lowered to the floor or there's a change in plane, it is considered a fall. The DON said her expectation was that both CNA #1 and CNA #2 should have called for the nurse prior to getting Resident #1 up off the floor and transferring Resident #1 onto the toilet.
