**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on

observation, interview, and record review, the facility failed to provide foot care and treatment in accordance with professional standards of practice for a resident with Diabetes. The facility failed to schedule and complete podiatry appointments for toenail evaluation and care for 1 (#11) of 5 (#9, #11, #56, #64 and #149) residents sampled for Activities of Daily Living (ADLs). Review of the Medical Record for Resident #11 revealed the resident was admitted to the facility on [DATE REDACTED] with diagnosis, which included Type 2 Diabetes Mellitus.Review of the most recent MDS (Minimum Data Set) for Resident #11 with an ARD (Assessment Reference Date) of 07/23/2025 revealed Resident #11 had a BIMS (Brief Interview for Mental Status) of 12, which indicated the resident was moderate cognitively impaired. Further review revealed Resident #11 required supervision for bathing. Further review of Resident #11's medical record revealed no documentation of a Podiatry referral nor nail care provided by a Podiatrist.An observation was conducted of Resident #11 on 08/11/2025 at 9:30 a.m. Abnormally thickened toenails was noted on both big toes.

Resident #11 stated she had discomfort when the bedsheets were against her toes and wearing shoes.

She stated when her friends visited they brought nail clippers for her to trim her own toenails. She stated

she requested to see a podiatrist since January 2025 but still had not seen a Podiatrist. An interview was conducted with S18DSS on 08/13/2025 at 11:40 a.m. She stated they contracted with an outside provider for podiatry and they conducted visits at the facility every 3 months. She stated a resident, nurse, or CNA can make a request for the resident to be evaluated by the podiatrist. She stated once she received the request, she notified the outside provider and the outside provider would reach out to the resident or RP to get the services started. She confirmed Resident #11 and Resident #11's daughter requested to be evaluated by the Podiatrist but could not recall when the request was made. She reviewed Resident #11's chart her emails and confirmed she did not have any documentation of the request. She confirmed she received an email from the daughter on 07/19/2025 but Resident #11 requested an appointment prior to receipt of the email. She confirmed the contracted provider did not have any documentation Resident #11 wanted to begin the services. An interview was conducted with Resident #11's daughter on 08/13/2025 at 3:03 p.m. She stated her mother had been asking since January 2025 to see a podiatrist and after months of Resident #11 requesting to see the podiatrist, she emailed S18DSS on 07/19/2025. She stated S18DSS responded and informed her Resident #11 needed a diagnosis of DM or PVD. Resident #11's daughter stated she informed S18DSS that Resident #11 did have a diagnosis of Diabetes. An interview was conducted with the S2DON on 08/13/2025 at 3:45 p.m. She S18DSS would be notified of a residents request to see the podiatrist and S18DSS should notify the contracted provider to begin services on the next visit.
