Inspection Findings
CNA was observed to perform hand hygiene during Resident #2's incontinence care. On 03/25/2026 at
gloves after removing bowel movement from Resident #1 and #2 and prior to moving to a clean area.

On 03/25/2026 at 9:15 a.m., an interview was conducted with S3CNA. S3CNA confirmed she should have performed hand hygiene and changed gloves after she wiped Resident #2 and before she put the clean diaper back on. S3CNA confirmed she did not perform hand hygiene upon removing soiled gloves and exiting Resident #2's room and should have. On 03/25/2026 at 12:35 p.m., an interview was conducted with S1DON.
She confirmed staff should change their gloves when soiled and when moving from a contaminated to a clean area during resident care.
