all-staff in-service was conducted. Resident R1's sibling and the responsible party did not want Resident R1 to use the electric wheelchair, which was discontinued on 09/04/25. Administrative staff B stated staff care planned Resident R1 as at moderate risk for elopement, noted all doors had key pads, and Resident R1 was not able to exit on her own without
a power chair. The facility's Resident Elopement Policy and Procedure policy, revised 12/2022, documented
the facility's intent to promote a safe and secure environment to help minimize the risk of residents leaving
the premises or a safe area without the necessary supervision or authorization to do so. Residents are to be assessed for elopement risk within the first 24 hours of admission, quarterly, and annually, for behaviors indicating potential elopement risk and with any significant change. Assessments are to be documented in
the clinical record. If an identified risk is present, the interventions implemented are to be documented on
the resident's care plan. On 09/23/25 at 01:59 PM, Administrative Staff A was provided the IJ template and notified of the facility's failure to prevent a cognitively impaired resident from leaving the facility grounds without staff supervision. The residenet fell while out unsupervised and sustained a head injury, placed Resident R1
in immediate jeopardy.The deficient practice was determined as Past Non-Compliance on 08/25/25 when
the facility implemented the following corrective actions:Notifications made to the resident's PCP and DPOAStatement obtained from staff members (see witness statements, Emergency QAPI meeting held with Dr [NAME], Medical Director, Director of Nursing, and ED on 08/25/25 Emergency resident council meeting on 08/25/25Staff meeting held on 08/25/25, on the elopement policy and procedure.
