order in effect.On [DATE REDACTED] at 02:41 PM, Administrative Staff A received a copy of the Immediate Jeopardy [IJ] Template and was informed that the facility's failure to ensure staff provided CPR to Resident R1, who desired resuscitative measures, placed Resident R1 and all 61 residents with full code status in IJ.The facility identified and completed the following corrective actions by [DATE REDACTED]:1. The nursing staff were re-educated on residents' rights, including the right to self-determination and to formulate advanced directives, education on advanced directives, including DNR orders and DNR directives, Full Code status, and CPR protocols.2. All licensed nurses demonstrated competency with the Code Status written quiz and Code Blue Protocols, including return demonstration.3. Mock Code Drills were completed two to four times a week.4. The Annual Education Calendar was updated to include code blue protocols and honoring code status.5. The facility added code blue response protocols to be demonstrated upon hire during clinical competencies.On [DATE REDACTED],
