Inspection Findings
not believe she entered a progress note after she assessed Resident B.
There should have been a
LPN 12 indicated she was in the dining room with LPN 11 when Resident B started coughing. LPN 12
food. LPN 11 asked staff members to get him thickened liquids. He drank the thickened liquids and spit it up as well. LPN 12 felt Resident B had food in his throat. He was still coughing but was able to breathe. He communicated by shaking his head yes and no.
Every time Resident B tried to talk, there was food in his mouth that would come out. LPN 11 propelled him out of the dining room to be assessed by LPN 4.
The Heimlich maneuver was not used on Resident B.On 2/20/26 at 11:58 a.m., the DON indicated there should have been a progress note in the EMR regarding Resident B's possible aspiration. If not a progress note, at least an SBAR or a documented event.
The next shift's nurse should have taken Resident B's vital signs and placed a progress note in the EMR regarding the resident's assessment.On 2/20/26 at 12:00 p.m., the Corporate Consultant indicated staff should have started a hot charting event or an infection control event.
The nurse would make a progress note under the open event.
She felt LPN 4 did more than what was documented.
After the hot charting was opened, on 2/13/26 at 2:41 p.m., regarding Resident B having started on an antibiotic, the following shifts should have documented Resident B's assessments in their progress notes. A current facility policy, dated 8/1988 and revised on 11/2018, titled Resident Change of Condition Policy, provided by the Administrator, on 2/20/26 at 3:30 p.m., indicated the following: .Life Threatening Change: D.
All nursing action, physician contacts, and resident assessment information will be documented in the medical record.
