Potential for More Than Minimal Harm

During an interview, on 11/6/25 at 4:15 p.m., the Executive Nurse Consultant (ENC) indicated the call lights should have been answered timely and care should have been provided. A privacy curtain should have been available for use.

During an interview, on 11/7/25 at 12:53 p.m., Resident T indicated when her call light was answered on 11/6/25 at 3:44 p.m., she had requested be assisted back to bed.

After her call light was answered, Resident T waited an hour and a half before being assisted to go to bed.

Resident T indicated that it was too long to wait and waiting that long had caused her to be in some pain. On 11/7/25 at 1:33 p.m., the Executive Director (ED) provided the Resident Rights- Quality of Life policy, revised 8/2020, that read .Each resident shall be cared for in a manner that promotes and enhance the quality of life, dignity, respect and individuality . III.

Residents are assisted in attending the activities of their choice. IV.

Resident' private pace and property are respected .VII.

Facility Staff speaks respectfully to residents at all times .IX.

Facility Staff shall maintain an environment in which confidential clinical information is protected .X.

Facility Staff promotes, maintains, and protects resident privacy, including bodily privacy, when assisting with personal care and during treatment procedures .On 11/7/25 at 1:33 p.m., the ED provided the Privacy and Dignity policy, revised 6/2020, that read .The Facility promotes resident care in a manner and an environment that maintains or enhances dignity and respect, in full recognition of each resident's individuality .
