cognitive impairment.A Physician's Order, dated 12/4/2024, indicated the resident was to receive losartan 100 milligrams orally daily. There were no administration parameters ordered for the administration of the losartan.The following blood pressures were obtained, the losartan given and the physician was not notified of the abnormal blood pressure readings.-7/1/2025 8:40 A.M. 113/55 mmHg-7/31/2025 9:45 A.M. 110/41 mmHg-8/11/2025 9:48 A.M. 123/41 mmHg-8/23/2025 9:10 P.M. 122/49 mmHgA Care Plan, initiated 11/9/2020 and revised on 5/21/2024, indicated Resident F had hypertension. Interventions included, but were not limited to: give antihypertensive medications as ordered and monitor for side effects of orthostatic hypotension.During an interview, on 8/27/2025 at 2:09 P.M., the DON indicated based on nursing judgement, she would have held the administration of the losartan for a systolic blood pressure less than 130 mmHg and a diastolic blood pressure less than 90 mmHg. She indicated every nurse was different regarding their nursing judgement. During an interview, on 8/27/2025 at 2:42 P.M., Resident F's nurse practitioner indicated she would not be cool with the losartan being given with the diastolic blood pressures

in the 40's and 30's and was not aware the medication had been given with those blood pressure readings.A policy was requested for administration of antihypertensive medications, on 5/27/2025 at 3:00 P.M. The Executive Director indicated a policy was not available for administration of antihypertensive, but

the staff should have followed the physician's orders. This citation relates to intakes 2562332 and 2579643.3.1-37(a)
