SUMMARY STATEMENT OF DEFICIENCIES

resident had a fall, the nurse was to assess the resident for injury and provide first aid.

Resident's with unwitnessed falls, were to have neuro checks started and completed per policy.

Documentation in the resident's medical record should include a description of the occurrence, including time and place; physical and mental status of the resident; time of physician notification and physician response/orders and time of notification to resident family/representative.

Documentation of the physical and mental status of the resident involved would be completed each shift over at least the next 72 hours or until the resident's condition improves.

The occurrence would be communicated shift to shift as part of the report until the resident is stabilized or at least 72 hours post fall.

All falls would have a site investigation by appropriate staff completed to define the root cause of the fall which will help provide information to roll out interventions to prevent another fall from occurring.

Each fall needed a new care plan intervention.

Based on the results of the incident/accident/fall, the resident's care plan would be addressed to ensure that any needed points of focus had measurable goals with appropriate interventions in place.This Citation relates to Intake 2663465.3.1-45(a)

SUMMARY STATEMENT OF DEFICIENCIES

Based on the results of the PT/INR lab results, changes to the residents Coumadin dose would be made, documented in the nurse notes, and initialed when given in the MAR.On 11/12/25 at 3:45 P.M., the Director of Nursing indicated PT/INR labs done by facility staff using their machine, was documented in a log book kept at the nurses station.

When asked, she indicated the logs with PT/INR lab results were not part of the residents clinical record but should be.A current facility policy, titled Guidelines for Assessments was provided on 11/12/25 at 4:20 p.m. by the Regional Nurse Consultant (RNC).

The RNC indicated the policy referred to all documentation required to be in a resident record.

The policy indicated assessments/resident records were to be completed timely and documented accurately and thoroughly.

The resident record was to contain all information from facility staff and contracted qualified health professionals involved in a residents care.This Citation relates to Intake 2663465.3.1-50(a)
