11/19/25 at 12:14 PM, V2, Director of Nursing (DON), said that during care, the CNA (V7) had Resident R1 turned to her side and Resident R1 kept shifting herself. The CNA asked Resident R1 to stay still so she could clean her, but the resident shifted again and started to slowly slide down the bed. The CNA grabbed the sheet to keep the resident from falling, but couldn't stop the fall, so the CNA lowered Resident R1 slowly to the floor. V2 said the ADON (V3 -Assistant Director of Nursing) spoke to Resident R1 and they updated her care plan and added education on body position and alignment in bed.On 11/19/25 at 12:23 PM, V7 said she was changing Resident R1 and had Resident R1 turned to her right side, and she was behind her on the left side of the bed. V7 said Resident R1 was scooting toward

the edge of the bed, and she told Resident R1 to stop scooting because she was going to fall, but Resident R1 scooted a little and she could see Resident R1's legs going over the edge of the bed. She reached for Resident R1 to keep her from falling, but couldn't get her stopped in time, so she grabbed her by the waist and eased her down towards the floor and Resident R1 ended up on the floor. V7 said Resident R1 can help turn her upper body but needs help crossing her (left) leg over to turn her lower body. V7 said if there were two people in the room, it would have prevented the fall. V7 said after Resident R1 fell, she now uses two people to turn Resident R1. On 11/19/25 at 2:38 PM, V6 said V7 yelled for help from Resident R1's room. When she entered Resident R1's room, she noted Resident R1 on the floor. V6 said V7 told her she was changing Resident R1 and told Resident R1 to roll over to her side, then her legs went too far over the side and Resident R1 started to slide down, so V7 grabbed the sheet to stop the fall, but wasn't able to, so she guided Resident R1 and the sheet to the floor.
