company treated the area when they returned for their next visit at the facility. On 3/27/26 at 10:49

Surveyor, V8, and V9 observed one of two dumpsters with the lids open, and large amounts of overflowing trash, and garbage surrounding the dumpster area. V8 stated the dumpster should remain closed at all times to avoid attracting rats and roaches to the facility. V9 stated it is the responsibility of the housekeeping department and the kitchen department to maintain the dumpster area. V8 stated, Oh we don't have it. (V1 Administrator) said she lets the pest control company know where to visit when we inform her of the sightings.On 3/27/26 at 2:20 pm, V1 (Administrator) stated R1 has reported to his nurse regarding roaches in his room. V1 stated she is notified by nurses or supervisors regarding any sightings of roaches, and any other pest in the facility. V1 explained once she is notified regarding a sighting of a roach or any other pest, she relays the information to the pest control company who will come within a few days and treat the area. V1 explained the pest control company is scheduled at the facility bimonthly and as needed. V1 explained the last report of a resident reporting roaches in the facility was on 3/23/26 and the pest control company treated the reported area for roaches on 3/25/26. V1 stated it is important to eradicate the facility of roaches in order to provide a homelike environment and promote a safe and clean safety standard.

The facility's undated policy titled Safe Handling Dumpster documents, in part: Policy: All food will be handled safely and disposed of in a safe manner.

Procedure: Dietary trash will be disposed of in a sealed plastic trash bag.

The sealed bags will be disposed of in the outside dumpster.

The dumpster will be on a solid surface.

The ground surrounding the dumpster will be free of trash and debris.

The facility's undated policy titled Pest Control Policy documents, in part: Purpose: To prevent or control insects and rodents from spreading disease.

The Environmental Service Director will be responsible for coordinating the facility pest control program .
