Administrative stated Staff A was not scheduled to work that weekend, she was off. She added the facility did reimburse him the $112 and gave him a lock box for his room, to store his wallet in.On 9/24/2025 at 1:40 PM attempted to call Staff A on the number provided in her employee file. An automated recording stated the number requested cannot be dialed then hung up. At 1:42 PM an email was sent to one of the two emails on file. On 9/25/2025 at 12:57 PM and 12:59 PM an email was sent to both email addresses on file.On 9/24/2025 at 3:35 PM the Administrator was asked if the facility had a statement from Staff A and

she indicated Staff A had denied the whole thing. During a follow up on 9/25/2025 at 3:33 PM the Administrator stated the Assistant Administrator contacted her to notify her that while she went to check on Resident #1 she noticed Staff A was in Resident #1's room. She stepped out, then went in after Staff A left to see what had went on. Resident #1 told her Staff A was asking for money. When the Assistant Administrator asked more questions, Resident #1 told her he had given Staff A money before. He said he had given Staff $100 previously and $12 on that Sunday. He told staff he had lent her the money thinking Staff A would pay him back. He told the Administrator Staff A tried to use the ATM at a gas station but it was not working then told him she would pay him back on pay day Friday, but she never did. Resident #1 stated that if she did not pay him back, he would consider it a wash. Resident #1 said Staff A was complaining about her car battery not working and needed to replace it so she could show up to work. He admitted to

the Administrator that he offered to give Staff A money the first time, but the second time she was seeking it out. The second time he gave Staff A, he stated she told him she was leaving her home because of a domestic situation. The Administrator stated they reimbursed Resident #1 the $112 he gave to Staff A and gave him a lock box to place his wallet in, in his room. When they interviewed other residents, no other resident had given Staff A or other staff members money. The Administrator indicated Resident #1 is his own payee, he pays his client participation by check every month and keeps what money he has left over.

She added he has a bank account and did not normally keep cash, like that, on him. When they tried to talk with Staff A, she told staff that she came to the facility that weekend she was on the schedule to say bye to her favorite residents. She gave them a list of residents she spoke too but those residents had low BIMS scores and could not recall anything. When asked if Staff A would come in on her days off, the Administrator stated she has been in the building a few times on her days off. The Administrator stated they immediately educated all staff members that it's not acceptable to ask or take money from residents even if

the residents has them to go buy them something from the vending machine. Staff are to go to management so it can be handled properly.On 9/29/2025 at 10:17 AM Staff A stated she told the facility the allegation was false but told them to do their investigation anyways. She stated she did not ask for money nor did she take money from Resident #1. When asked when a good time would be to set up a time and place to meet to discuss this further she indicated she did not have a car and lived 2 miles away from town to meet anywhere.The facility provided a document titled Abuse, Neglect, Exploitation and Misappropriation Prevention Program with a revision date of April 2021. The policy statement included: residents have the right to be free from abuse, neglect, misappropriation of resident property and exploitation. The resident abuse, neglect and exploitation prevention program consists of a facility-wide commitment and resource allocation to support the following objectives:1.
