Staff C asked her to go down and use the restroom and she headed down to her room around 6:05 p.m.

Resident #1 came back up to the nurse's station and asked for her family member's number. Staff C told her there was a piece of paper in her room and Resident #1 went to look for it (this was usually a good distraction and focus point for her) and Staff D Licensed Practical Nurse (LPN) said to Resident #1 why don't she go look for it. Staff C didn't see Resident #1 return to the desk right away, so at 6:15 p.m. she went down the hall to help with a resident transfer. She learned of Resident #1 (eloping) around 6:45 p.m.

They immediately initiated frequency checks. Throughout the shift Staff C used several interventions to assist with Resident #1's wandering and behavior. These included offering food, offering fluids, offering toileting, activity provided, and assessed for pain. Except for at 3:35 p.m. when she asked to go down the hill to see her family, Resident #1 did not display any exit seeking behavior or indications that she wanted to leave. Staff C signed the statement on 8/31/25.During an interview on 10/21/25 at 2:15 p.m. the Administrator stated since the incident on 8/30/25 they had investigated what happened. The door alarm apparently was off when the incident occurred. She had interviewed staff and visitors and was unable to determine who/how the alarm was turned off. They did not feel Resident #1 was outside very long, or if she even went clear outside. Based on the timeline there had only been 10 minutes since she had last been seen.The facility Missing Resident/Elopement Process updated 10/29/24 included when the door alarm sounded, facility staff should: a. Reset the door alarm after they determined by visual check that no resident exited the facility inappropriately or returned to the facility. b. If for any reason, door alarms were turned off,

the staff would continually visually monitor the door/doors. c. The nurse, Director of Nursing (DON), or Executive Director would question staff to determine who deactivated the door alarm and reason for doing so.
