reported the facility terminated the employment of Staff A and Staff B as a result of their failure to use the stand lift as the facility expected.The DON reported the fall and fracture for Resident #1 was preventable.

The DON stated if a resident refused to use safety items, she expected the staff to let the nurse know. On 12/23/25 at 2:20 the Administrator reported she expected the CNAs to transfer Resident #1 with the stand lift with the belt/straps in place. The Administrator revealed the fall and fracture for Resident #1 was 100% preventable. The Administrator reported both the Staff A and B were terminated. The facility provided a Lifts Program Skills Check-Off Sheet for Stand Assist Mechanical Lifts dated 7/2018, directed fasten the safety belt around the resident's waist. The facility provided a lift Instruction, undated, included the following: Standing Clip Sling is intended to be used together with a lifting device to provide body support during raising to a standing position and short transfers of adult patients/residents. Applying the Sling directed place the sling around the patient's lower back .Make sure that: patient's arms are outside the sling, support belt is positioned loosely around the patient's body and support belt is not twisted or trapped behind the patient's back .Make sure you place the leg flaps around the legs and these are not twisted. Fasten the support belt securely by pressing the buckles or hook and loop attachments together. The support belt should be tight but comfortable for the patient.
