observations, interviews and record review, the facility failed to accommodate the needs of one of one resident [Resident (R)78] sampled by not ensuring that the call light was always placed within reach or positioned so that it could be activated. As a result of this deficient practice, Resident R78 was placed at risk of not having his emergent needs met in a timely manner. This deficient practice has the potential to affect all the residents at the facility who can activate a call light.Findings include:Resident R78 was a [AGE] year-old resident admitted to the facility on [DATE REDACTED] for long-term placement. Diagnoses included, but not limited to, quadriplegia (paralysis in all four limbs and torso) and has a tracheostomy (surgical opening in the front of

the neck where a tube is inserted into the windpipe to maintain an airway). Recent Minimum Data Set (MDS) quarterly assessment on 08/02/2025 revealed Resident R78 had a Brief Interview for Mental Status (BIMS) score of 15, indicating he is cognitively intact.On 09/02/25 at 10:21 AM, observed Resident R78 lying in bed watching television. Call light was placed on the mattress, left side of Resident R78's pillow and out of his reach.

Asked Resident R78 if he was capable of using the call light if he needed assistance from the staff. Resident R78 said, Yes, if I can reach it with my head. Resident R78 asked the surveyor to activate the call light so staff can pin it to the pillow and closer to his head. One minute after activating the call light, Certified Nurse Aide (CNA)122 knocked at

the door and asked if Resident R78 needed assistance. Resident R78 asked CNA122 to position the call light closer to his head so he can reach it. CNA122 acknowledged the call light was out of Resident R78's reach and proceeded to pin

it on the left side of the pillow.On 09/03/25 at 09:19 AM, observed Resident R78 lying in bed with his eyes closed.

Call light was placed on the mattress near the pillow and out of Resident R78's reach.On 09/05/25 at 10:11 AM, an

interview was conducted with the Nurse Supervisor (NS) 3 in front of the nurse's station. Shared with the NS3 observations of Resident R78's call light being out of his reach. NS3 said staff are to make sure Resident R78 is able to reach his call light at all times since he is able to activate it in case he needed assistance.
