According to the January 2025 CPO, diagnoses included drug induced dyskinesia (uncontrolled involuntary muscle movement), dysphagia (difficulty swallowing) and dementia.

The 11/27/24 MDS assessment revealed the resident had a severe cognitive impairment with a BIMS score of five out of 15.

The resident ate and walked independently and he required set-up assistance with showering.
