residents will receive behavioral health services as needed to attain or maintain the highest practicable physical, mental and psychosocial well-being in accordance with the comprehensive assessment and plan of care.Behavioral symptoms will be identified using facility-approved behavioral screening tools and the comprehensive assessment.Behavioral health services will be provided by qualified staff who have the competencies and skills necessary to provide appropriate services to the residents. Residents will have minimal complications associated with the management of altered or impaired behavior.The facility will comply with regulatory requirements related to the use of medications to manage behavioral changes.Assessment.As part of the initial assessment, the nursing staff and Attending Physician will identify individuals with a history of impaired cognition, altered behavior, substance use disorder, or mental disorder.All residents will receive a Level I PASARR screen prior to admission.If the level I screen indicates that the individual may meet the criteria for a mental disorder, intellectual disability or related condition he or she will be referred to the state PASARR representative for the Level II (evaluation and determination) screening process.The Level II evaluation report will be used when conducting the resident assessment and developing the care plan.New onset or changes in behavior that indicate newly evident or possible serious mental disorder, intellectual disability, or a related disorder will be referred for a PASARR Level II evaluation.Current Level II residents will be referred for an additional PASARR Level II evaluation upon a s
