Resident 1's RP had mentioned before and after the overdose incident that Resident 1 required a monthly injection for substance abuse cravings and that the DON had agreed to follow up with the facility's physician. DON acknowledged there was no documentation that the physician was informed, or that the physician provided any orders or guidance. In addition, the DON stated there is no evidence in Resident 1's

record the facility took any actions to provide the resident with a monthly injection to curb substance abuse cravings. During a review of the facilities policy and procedure (P&P) titled, Care Planning dated 8/25, the P&P indicated in part, It is the policy of this facility that the interdisciplinary team (IDT) shall develop a comprehensive Person-Centered Care Plan for each resident based on resident's needs to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being.Based on the interdisciplinary review, the care plan will be implemented, with ongoing revisions as needed.An interdisciplinary approach to individualize the care plan based on specific resident's needs will be an ongoing goal. During a review of the facilities P&P titled, Facility Prohibition Against Recreational Drugs, Substance and Alcohol Abuse, dated 8/25, the P&P indicated in part, It is the policy of [Facility Name], to provide and maintain a safe, efficient, and drug and/or alcohol-free living environment, conducive to the health, safety and well-being of our resident community.Recreational drugs and alcohol are inconsistent with the goal of promoting the health and rehabilitation of each individual resident and are expressly prohibited. Use or possession of, or being under the influence of, alcohol or recreational drugs by any resident of or visitor to the Facility necessarily presents a real and present danger to the health and safety of the general resident community.
