The DON also stated that not maintaining good toilet hygiene, the resident could be at risk for skin breakdown leading to inflammation, pain, discomfort, and UTI. During a
review of the facility's Policy and Procedures (P&P) titled Activities of Daily Living (ADLs), revised 3/2018,
the P&P indicated that residents who are unable to carry out activities of daily living independently will receive the services necessary to maintain good nutrition, grooming, and personal and oral hygiene. The P&P also indicated that appropriate care and services will be provided for residents who are unable to carry out ADLs independently, with the consent of the resident and in accordance with the plan of care, including appropriate support and assistance with elimination (toileting).

required frequent visual checks, however there was no documentation of frequently checking. DON stated that all residents who are at high risk for fall shall have the bed kept in low position. DON also stated per policy falls were defined unintentionally coming to rest on the ground, floor, or other lower level. A fall with no injury is still a fall. DON stated LVN and RN should have assessed Resident 3, done body checks right away when they were informed that the resident was found on the floor, and notified her provider and responsible party immediately. During a review of the facility's policy and procedures (P&P) Fall and fall Risk, Managing dated 3/2018, the P&P indicated the following: Based on previous evaluations and current data, the staff will identify interventions related to the resident's specific risks and causes to try to prevent
the resident from falling and to try to minimize complications from falling. Unintentionally coming to rest on
the ground, floor or other lower level. An episode where a resident lost his/her balance and would have fallen, if not for another person or if he or she had not caught him/herself, is considered a fall. A fall without injury is still a fall. Unless there is evidence sug-gesting otherwise, when a resident is found on the floor, a fall is considered to have occurred. During a review of the facility's policy and procedures (P&P) Safety and Supervision of Residents dated 7/2017, the P&P indicated the following: Our facility strives to make the environment as free from accident hazards as possible. Resident safety and supervision and assistance to prevent accidents are facility-wide priorities. Resident supervision is a core component of the systems approach to safety. The type and frequency of resident supervision is determined by the individual resident's assessed needs and identified hazards in the environment.
