LVN 1 stated valsartan should have been administered on 11/3/2025, at 9 a.m. LVN 1 stated Resident 1's blood pressure could increase because valsartan was not administered.During an interview on 11/20/2025, at 10:30 a.m., with LVN 2, LVN 2 stated on 11/6/2025, at 9 a.m., Resident 1's blood pressure was 106/58 mmHg. LVN 2 stated she (LVN 2) should have administered valsartan as per physician order. LVN 2 stated she (LVN 2) was used to following a physician order to hold the valsartan for sbp less than 110 mmHg. LVN 2 stated she (LVN 2) should have read the physician order and administer valsartan to Resident 1. LVN 2 stated Resident 1 could experience HTN because valsartan was not administered.

During an interview on 11/20/2025, at 10:23 a.m., with the Director of Nursing (DON), the DON stated valsartan should have been administered on 11/3/2025, and 11/6/2025, at 9 a.m. as per physician order.

The DON stated LVN 1 and LVN 2 should have read the physician order, compare with the blood pressure result and follow the physician order to administer the medication.During a concurrent interview, and record review on 11/20/2025, at 10:26 a.m., facility's policy and procedure (P&P), titled, Administering Medications, dated 3/2023, and last reviewed on 3/21/2025, the P&P indicated, Medications are administered in a safe and timely manner, and as prescribed.4.

Medications are administered in accordance with prescriber orders, including any required time frame.10.

Allergies to medications; andb.

Vital signs, if necessary.
