Inspection Findings
were no injuries noted. An interview with Administrator (ED/Staff # 100) on November 13, 2025 at 3:30 p.m. revealed that the process for resident-to-resident abuse is the staff member present at the time of the altercation will notify a supervisor and that supervisor will contact the Director of Nursing or the Administrator. ED # 100 also reveals that residents are separated and she makes the notifications to
reveal that an investigation is conducted and the results of the investigation are sent to DHS. A Policy and Procedure titled, Abuse and Neglect adopted on May 1, 2024 revealed that it is the policy of this facility to provide professional care and services in an environment that is free from any type of abuse.
The policy goes on to define abuse as the willful infliction of injury.
Physical abuse includes but not limited to infliction of injury that occur other than by accidental means such as punching, hitting, kicking, and slapping.
