Regarding the incident with resident #10, the DON said that something broke down with communication and the resident's code status was misread and miscommunicated. The facility policy, titled, Advanced Directives Policy with an implementation date of November of 2024 included that: the facility will adhere to residents' rights to formulate advanced directives.
The facility will implement procedures to communicate a resident's code status to those individuals who need to know this information. The policy also included that when an order is written pertaining to a resident's presence or absence of an advanced directive, the directions will be clearly documented in designated sections of the medical record. The nurse who notates the physician order is responsible for documenting the directions in all relevant sections of the medical record. The designated sections of the medical record are at the nurses station/unit.
