The maintenance manager, identified in the inspection report by his title, told inspectors that whoever used the dumpster was responsible for making sure the trash made it inside and the doors got closed afterward. He said staff who didn't do that should be held accountable. Then he acknowledged he himself had not been trained on waste disposal and had never come across the facility's waste disposal policy. He said the open doors and ground-level trash "could cause the spread of rodents and environmental pollution."
The administrator's answer was similar. He said dumpster monitoring was typically handled through what he called an "IDT approach," meaning an interdisciplinary team. At the time of the inspection, he said, no one had actually been assigned to carry out that monitoring. He could not explain why the iron bars and black trash bags had been left on the ground, attributing it to "lack of coordination." He added that staff had not been trained on waste disposal tasks and had not come across the policy either.
The policy existed. Inspectors reviewed a facility document titled "Dumpster Use and Waste Disposal Policy," though it carried no date. The policy stated that all waste must go directly into designated dumpsters, that nothing should be left on the ground or beside them, and that dumpster lids and access doors must remain closed when not in use. It also required staff to routinely inspect dumpster areas and keep them clean and free of pests and safety hazards.
None of that was happening.
The administrator noted one specific concern about what the failures meant for residents: staff who handled trash and then returned to resident care areas without proper hand hygiene created an infection risk. He described that as a "potential infection risk" in his own words.
CMS cited the violation at a level of minimal harm, with some residents affected. The citation focused on the risk that open dumpsters and ground-level waste posed for vermin and rodents, which carry disease, and the potential for that exposure to reach residents inside the facility.
