Housekeeper T told inspectors on January 28 that he poured enough Medorra Limpreza All Purpose Cleaner Lavender scent "to cover the bottom of the bucket" when mopping floors. The facility previously had a system that automatically mixed water and cleaner through a hose, which the housekeeper preferred.

The cleaner bottle had no EPA-registered number. Inspectors could not find the product listed on EPA.gov when they searched on February 6. The Medorra Limpreza official website showed a "critical error" when inspectors tried to verify the product or its EPA registration on multiple dates in late January and early February.
Supply problems began when new management took over in September 2025. Central Supply worker PPP, who has worked at the facility for several years, said the change was "drastic." Before the management transition, "there was never a problem with supplies," but afterward budget issues left the facility with "limited supplies."
PPP's job expanded in October to include medical records and accounts payable duties. "He tried to do everything, but it is a lot," according to the inspection report.
The Regional Nurse Consultant confirmed that housekeeping staff order their own supplies and that the facility has no Housekeeping Director. Staff received no training on proper amounts of floor chemicals to use.
Administrator B acknowledged there were extra supplies stored in the basement and said he told staff to educate someone on where to find them. He expected housekeeping to use appropriate cleaning supplies and to know proper chemical amounts without formal training.
The facility's corporate structure limits local control over supply ordering, according to the Dietary Manager, who said "they had a corporate person that ordered supplies for them."
