Housekeeper T told inspectors on January 28 that he poured enough Medorra Limpreza All Purpose Cleaner Lavender scent "to cover the bottom of the bucket" when cleaning floors. The employee said they used to have a hose system that automatically measured water and cleaner, and "liked that set up better."

Inspectors found no EPA-registered number on the cleaner bottle. A search of EPA.gov showed no results for Medorra Limpreza All Purpose Cleaner Lavender scent. When inspectors checked the Medorra Limpreza official website multiple times between January 28 and February 6, they found "a critical error" and could not verify the product or its EPA registration.
The cleaning product issues emerged during a broader supply crisis that began when new management took over in September 2025. Central Supply PPP, who worked in the building for several years, said the facility "went from having supplies to limited supplies" due to budget constraints under the new leadership.
"Before the management change, there was never a problem with supplies," the employee told inspectors. "When they took over, there was a budget issue."
Regional Nurse Consultant confirmed that housekeeping staff "did their own ordering" without a Housekeeping Director overseeing purchases. The consultant said there was "no training for staff on how much floor chemicals to use."
Administrator B acknowledged the facility had "extra supplies in the basement" and expected housekeeping staff to use appropriate cleaning products. The administrator said he expected employees to "know how much chemicals to use" without providing specific training.
The Dietary Manager told inspectors that a "corporate person" ordered supplies for the facility, limiting local control over purchasing decisions.
